Patient Forms
For your convenience, we have provided patient forms below. We encourage all patients to print out and complete the forms prior to your visit. Otherwise, please try to arrive 10-15 minutes prior to your appointment so the required paperwork can be completed. Please also bring these items with you to your appointment:
• Your current eye glasses or contact lens box with label
• A list of your current prescriptions and over-the-counter medications that you take regularly
• Medical insurance cards
• A referral from your medical doctor, if required by your insurance company
